Distal Fibula Physeal Fracture

Salter Harris I distal fibula fractures are the most common ankle fractures.

Unfortunately often missed or misdiagnosed as sprain.

Tenderness is directly over lateral malleolus not ligaments.

MANAGEMENT

  • Analgesia

  • Undisplaced SH I and II fractures.

    • Below knee backslab​

    • Non weight bearing

    • Fracture clinic

Minimally displaced SH 1 distal fibula fracture

REFER ORTHO if:

  • Salter Harris III or IV

  • Displaced and reduction not possible or adequate in the ED

  • Extreme swelling/compartment syndrome

  • Open Fracture

  • Neurovascular injury
