Tillaux Fracture

ASSOCIATED WITH

  • distal fibular fracture (usually SH I or II)

  • ipsilateral tibial shaft fracture

  • Salter-Harris type III fracture involving avulsion of the anterolateral corner of the distal tibial epiphysis

  • In children the growth plate is weaker than the anterior tibiofibular ligament and can become avulsed (in adults the ligament would give way first).

  • Tillaux fracture occurs in adolescents where the middle and medial parts of the epiphyseal plate have closed but lateral part is still open.

  • Age 12-15 years

MANAGEMENT

  • Analgesia

  • REFER ORTHO

  • Displaced >2mm

    • Operative management

  • Displacement <2mm

    • Above knee backslab

    • Non weight bearing

    • CT scan may be required to confirm not displaced.
